About:
This award recognizes quality improvement projects designed to improve safety and/or quality in practice. Up to two recipients will be selected each year. All are eligible to apply.

The application for this award includes a short form describing the patient safety or quality improvement project that was implemented. A current CV of the project lead as well as a one-page abstract is required. The one-page abstract should include 1) project participant name(s), 2) project title, 3) objective(s), 4) background, 5) methods, 6) data and results, and 7) conclusions.

Benefits:
The recipient will receive a $1,000 prize.
